Director of Hospital Case Management

Job Summary : As the Director of Case Management, youwill oversee and coordinate department activities, ensuringcompliance with organizational policies and external regulatoryagencies.

You will be responsible for managing all aspects of dailyhospital case management operations, integrating patient care anddischarge planning processes, and achieving established outcomesthrough interdisciplinary collaboration.

With 400+ bed acute care hospital features services such asa Chest Pain Center, Stroke Center, dedicated Heart Center, and astate-of-the-art Spine Clinic, also offering comprehensivematernity services.

They are a Certified Level II Trauma Center.Qualifications : - Bachelor's degree required; Master's degree inNursing, Health Administration, or Business Administrationpreferred.

• Either Registered Nurse (RN) licensure or LicensedClinical Social Worker (LCSW) required. - 5-7 years’ experience inacute care hospital case management.
• 2+ years’ experience in casemanagement leadership. - Experience in a leadership role in a largeacute care facility, preferably a Trauma Level hospital.
• Strongcommunication skills, ability to multi-task, and a polishedprofessional demeanor. Key Responsibilities : - Direct and evaluatedepartmental operations, including staffing, onboarding, and use ofinformation technologies.
• Allocate resources to meet productivityand quality goals. - Oversee core functions and practices of casemanagement. - Ensure adherence to care coordination and dischargeplanning processes.
• Collaborate with the Facility CFO andDivision CM Leader to achieve goals and expectations. - Assess andimprove department performance through data analysis andsustainable performance improvement activities.
• Ensure compliancewith policies and standard operating procedures. - Establishworking relationships with key stakeholders, including CMO, CNO,ancillary service leaders, Ethics & Compliance, and Legal.
• Perform other duties as assigned. - Adhere to the Code of Conduct and Mission and Value Statement.
